For example, in a cohort of skiers, Myrstad et al. showed that the risk of AF was heightened among skiers who completed a greater number of races and/or those who had faster finishing times. Furthermore, cumulative years of endurance exercise also have prognostic significance, with a 4-fold increased risk of incident AF observed above the threshold of 2000 h of lifetime endurance training.114,125–129 There are multiple mechanisms by which PA may modify risk of AF.

However, several studies described an increased risk and prevalence of AF in athletes [9–11]. The results of the main studies investigating the risk of AF in athletes are reported in Table 1 [12–27]. These conflicting results derive mostly from the design of the included studies.

Young athletes have a low prevalence (0.2%) of AF, despite echocardiographic evidence of atrial enlargement [11]; surveillance to the age of 40 years in another series also showed no excess AF [12]. Lifelong male endurance cyclists have both demonstrably higher prevalence of sinus pauses and atrial arrhythmias than non-endurance golfers (6% versus 0%) [10]; the epidemiology of AF burden remains undefined in female athletes although emerging evidence suggests a greater lifetime endurance training requirement of around 40 years may portend increased risk [13]. Pacemaker implantation was performed in 3 of 30 (10%) affected athletes over 9 years of an observational study [14], in excess of the 3% requirement in endurance athletes without premature onset atrial arrhythmias [10].

Studies explicitly investigating female athletes’ risk of AF are rare. Several studies in women have not shown an increased risk of AF but, rather, a decreased risk [26–28]. The level of exercise reported in these studies is variable.
