INTRODUCCIÓN
Durante décadas se ha subestimado la intervención del ventrículo derecho (VD) en la estabilidad hemodinámica y el mantenimiento de un adecuado gasto cardiaco. La evidencia científica, sin embargo, nos ha demostrado el papel primordial de la función sistólica del VD en la evolución de diversas afecciones. Se conoce desde hace tiempo su valor pronóstico en las cardiopatías congénitas, en las cuales su influencia se explica en gran parte por variaciones anatómicas que otorgan a las cámaras
MÉTODOS
Se incluyó de forma consecutiva a todos los pacientes ingresados en nuestro centro en espera de actuación quirúrgica aislada sobre la válvula mitral. Se excluyó a los pacientes con enfermedad coronaria o con valvulopatía aórtica concomitante.
Se empleó un equipo de ecocardiografía (General Electric, Vivid 3) dotado de un transductor de 1,5 a 3 MHz (Sonda 3S). El análisis de las imágenes se realizó en la estación de trabajo de nuestra unidad de imagen central con el programa informático Echo Pac
RESULTADOS
Se incluyó en total a 30 pacientes consecutivos entre octubre de 2006 y noviembre de 2007 que estaban programados para actuación quirúrgica mitral aislada en un único centro hospitalario.
La mayoría de los pacientes eran mujeres con una media de edad de 67 años. La etiología más frecuente de la valvulopatía mitral fue reumática. En la mayor parte de los casos, se presentó como insuficiencia mitral severa con hipertensión pulmonar severa. La función ventricular izquierda estaba conservada en la
DISCUSIÓN
Indudablemente, es preciso obtener información de la función sistólica ventricular derecha para la toma de decisiones clínicas y en la evaluación pronóstica de los pacientes con diferentes enfermedades cardiovasculares. Existen diversos métodos de imagen que evalúan la función del VD, de los que el más ampliamente utilizado en la práctica clínica habitual es la ecocardiografía bidimensional. Sin embargo, la evaluación ecocardiográfica del VD dista mucho de ser perfecta y no existe un parámetro
CONCLUSIONES
La TAPSE y VPDT son los parámetros ecocardiográficos con menor variabilidad inter e intraobservador, por lo que son los más fiables y reproducibles en la valoración de la función sistólica ventricular derecha en pacientes con valvulopatía mitral aislada.
