    As clinicians involved in developing the HealthPathways platform, we would like to applaud Tim Stokes et al. on their paper (Stokes T, et al. BMJ Open 2018;8:e025094. doi:10.1136/bmjopen-2018-025094). The authors examined the implementation of HealthPathways in the Southern District Health Board (DHB), in the south of New Zealand, using a qualitative methodology involving interviewing a small sample of participants in the development of the pathways. HealthPathways is a clinical guidance platform, intended for general practice audiences and designed in collaboration with general practice and hospital practitioners (https://www.healthpathwayscommunity.org/). The programme originated in the Canterbury region of New Zealand and has been adopted by 43 health regions in New Zealand, Australia and the United Kingdom, covering about 26 million people. We have been involved in implementation of all regions and thus feel able to compare the approaches in different places.
    We agree with the findings of Stokes et al. that just delivering the web portal is inadequate and the adaptable periphery (local context) is important to achieve maximum benefit for patients. The lessons learnt from the Southern implementation are very valuable. Notably, however,at the time of evaluation Southern had not delivered the full suite of locally adjusted pathways.
